Download and complete our application packet. This includes our counseling agreement as well as a Personal Data Inventory that will help us become familiar with the basics of your situation before our first appointment.

Mail or drop off the completed packet along with a $25 refundable deposit made payable to “The Bible Church of Little Rock.” This deposit will be returned when your counseling ends (for any reason) unless you have a last-minute cancellation of a scheduled appointment, in which case it is forfeited. Mail or deliver the application packet and deposit to The Bible Church of Little Rock, 19111 Cantrell Road, Little Rock, AR 72223.

Once your information has been received and verified, you will be contacted by one of our staff to schedule your counseling appointment.
